Please use the if your Head of School, location, or additional contact has changed since you submitted your previous year鈥檚 Private School Approval Application. Submitting this form alerts the State 91原创 of Education concerning changes at your school, which will ensure continuity of communication from state agencies.
Note: There is no need to submit a Change Alert form if you listed your new head of school or other personnel changes in your 2024鈥2025 Approval Application.
Important Notes:
- Processing:
Forms are reviewed twice monthly, around the 1st and 15th.
Changes to the head of school or contact information typically appear in within a week. Address changes may take longer.
- New Principals or Heads of Schools:
Visit the main and select "Create an Account.鈥
Contact OSPI Customer Support to request user roles. For more information, see EDS Access.
- School Name Changes:
These are only processed during the spring Approval Application period.
When completing the Renewing Private School Application, use the new name of the school.
- Form Access:
The form is disabled during the Approval Application window. Report changes for the upcoming school year directly in the Approval Application.
- Temporary or Permanent School Closures:
Submit the to notify the State 91原创 of your school's closure
